Gupta and Gupta Pediatrics Policy Reminder

Dear Patients and Families,
As we move forward into the 2024 year, we want to thank you for entrusting us with the care of your children. We value your family and are committed to their well-being. For our office to provide the best care for your family, we would like you to review the following administrative policies in place to provide the most effective and efficient care possible.

1. Shot records: We ask that you bring a copy of all vaccines given that were not administered in our office. This allows us to update your child’s records to ensure immunizations are ordered and administered correctly. A shot record may be requested at any time, with a nonrefundable fee of $5 cash or $6 credit card fee. We ask that you give us 24-48 hours for the request to be completed.
2. Insurance: Insurance should be updated and verified at every appointment. As per policy, for Medicaid patients with Commercial insurance we are required by State Law to bill the Commercial insurance first. Please ensure to provide all policies your child has; this will help us bill insurance on time and avoid an incorrect balance due on your account.
Medicaid patients must confirm that the PCP (primary care physician) is Dr. Sanjay H. Patel. Our staff is committed to your family and will notify you before your appointment if a PCP change needs to be made. We ask that the change be made before your appointment and a reference number obtained, failure to do so will result in a rescheduling of your appointment.

3. Refills: Requests for refills require 48 hours’ notice. We understand life may get in the way but please make it a habit to request refills when your child is a week away from their last pill to ensure they receive the medication in a timely matter. Follow up appointments are a requirement for refills on ADHD medication.
4. No Show Policy: We do our best to ensure that all patients and their families are reminded of each upcoming appointment. When a patient does not show up to their appointment, it takes away an appointment that could have been made for another child. We understand life happens, we ask for a 24-hour notice of cancellation. A $35 dollar fee may be assessed for no shows and will be required to pay before making your next appointment. More than three (3) no shows can result in a dismissal from our practice.
5. Pre-registration with Phreesia: Completion of the registration on Phreesia must be completed BEFORE your appointment to avoid rescheduling. If you are having trouble, please come in early and we can help.
